THE FOLLOWING ARE OUR POLICIES FOR SCHEDULING AND PAYMENT:
 

Appointments: 

  • Office hours are Monday - Friday, 9 AM to 5 PM.
     

Schedule of Fees for Services:

  • Consultations:

    • Adult (ages 16 and above): 2 hours $200

    • Pediatric (ages 1-16): 1.5 hours – $150

    • Infant (ages 0-1): 1 hour – $125

  • Integration Therapy:

    • Return Visit: 1 hour – $100

    • Return Visit: 30 minutes – $55

  • Email Exchange – $50

Email Service Policy:

  • I welcome clients to email me anytime with questions, concerns, or requests for resources. For more complex inquiries that require me to provide an in-depth response, it is my policy to charge $50 for such consultations. I will always let you know ahead of time if this fee applies. 

Home Visit Policy:

I offer home visits to make bodywork accessible to more individuals. Due to the time it takes for me to gather equipment from my office, travel to you, and set up in your home, I charge my hourly rate for travel, preparation, and breakdown time.

  • I will arrive for your appointment 15 minutes prior to the scheduled starting time. This allows me the time to prepare for the service and discuss your needs before we begin. All treatment sessions have a specific time schedule and early arrival allows for a relaxed and unhurried experience.

  • If I bring equipment (e.g. PEMF mat, vibration plate), I charge for breakdown, packing, and unpacking time.

  • I charge my hourly rate for breakdown, setup, and travel time. Please contact me if you would like me to price out the cost of bodywork sessions in your home prior to booking.

Weekend Policy:

I offer my services on weekends at an increased rate of $115 per hour. The reason for this is that weekends represent time for me to share with my loved ones. This time in my schedule is worth more to me, as I imagine it’s worth more to you.

 

Payment Policy:

  • Payment must be received before services are provided, otherwise the appointment will be cancelled.

  • Sessions are based on time spent with client and complexity of session and are subject to change accordingly.

  • Any bounced check is subject to the full amount of the check, plus fees up to $100.

  • Once a receipt of payment for services has been issued, whether issued electronically or in hard copy, any request for duplicate copy of receipt is subject to inconvenience fees of up to $50.

  • Terms of payment, fee schedules, and cancellation policy, agreed upon by a signed private contract between agencies or facilities and Jackson Penfield-Cyr, for private trainings, clinical consulting and business development, supersede the general payment policy as outlined above on this page.

Refund Policy

  • We have no refund policy on all services provided by Jackson Penfield-Cyr.

  • Any advanced payment for services not rendered will be refunded in full or partially refunded according to the term set forth below, unless the pre-paid consultation or package has expired according the terms of purchase below.

Cancellation and Rescheduling of Appointments Policy:

  • There is a 24-hour cancellation policy for any and all treatments (in-office or telephone visits), regardless of the reason for cancellation. This time is set aside solely for you, and I have made myself available to you for our work together. My full hourly fee is assessed for sessions cancelled within the 24-hour cancellation period. Please understand that I will not be able to support your healing in my practice if you are unable to agree and adhere to this policy.


Insurance reimbursement:

  • Upper Valley Integration Therapy does not bill insurance companies; however, our services may be covered in full or in part by your health insurance or employee health benefit savings plan. 

  • Receipts are available upon request, which you may submit to your insurer, your flexible savings account (FSA) or health savings account (HSA) for reimbursement.


Flexible Spending Plans & Health Savings Accounts:

  • Both Flex Spending Plans and Health Savings Accounts can be used to cover any visit fees not covered by your insurance.
